Strange behavior after logging into the OPAC

Koha 3.22.03 package install on Debian 8. The OPAC doesn't display the username when you are logged in. on the right side at the top it displays "Welcome, Search history [x] Log out". Then it displays "Hello, Click here if you're not" followed by the list of items I have checked out. It repeats everything in that block from Hello down for a total of 75 times. 500 error message appears when i add item with non latin content i. The field NOTE

Dear When i do add items to record with nonlatin content only in the Notes fields I got the message 500 error otherwise everything work perfectly, can someone help regarding this ?! Collection analysis according to subject

Hi good People, Am looking for an sql report that can help me analyse my collection according to specific subjects offered in specific programmes in the university.The report should be able to give me bibs and items attached to each bib. Dear all, After restore fresh koha db dump to old dump SYSTEM IS UP AND RUNNIG well. But after create new patron and when I try to login to opac using new patron u/n and p/w its getting error saying saying " Access denied Sorry, the system doesn't think you have permission to access this page. Pls help me to rectify this eror.
