[Koha] Cart icon - how to turn on?

We set the preference opacbookbag to "on" but the cart still does not appear. The option to add books to the cart gets turned on under each book record, but there is no cart icon to click on so that users can look at the contents of their cart. Are there additional steps we need to take to get the cart icon to display? I checked the user-configurable opac selections in the admin interface, such as opacheader and opacuserjs to see if there was some specific code suppressing the cart there, but couldn't find anything.
