The biggest blocker for Course Reserves, in my opinion, is Hourly Circulation. This needs to be completed first, since for many academic institutions, the loan period on a reserve item is less than a full day. There is an RFC on the wiki ( http://wiki.koha-community.org/wiki/Hourly_Loans_RFC), and while it is not currently funded, that's being worked on. Hopefully it'll be ready for Koha 3.4, but it's a large development, so perhaps not. Speaking as the RM for the next release, which is due in april 2011, it is quite unlikely that one will make it in for this release. As far as I know, no one is currently working on it. Hopefully someone will get funded to work on it and to contribute it back to Koha as the original implementation was supposed to be.
Someone might pick up the code that was released as Harley and spend some time bringing it up to a state where it could be interested, and if understand Daniel, fix the issues with it. Certainly if a working implementation that passed QA and applied cleanly to the master branch of the Koha reposity was submitted, then it would be considered for 3.4.
