Depending on the customer's email host, and your ISP's terms and restrictions, your emails may be getting flagged as spam or otherwise blocked before they can reach the customer's inbox. You may want to start by entering your server's IP into Spamhaus.org, and see what they have to say about it: http://www.spamhaus.org/lookup.lasso
And that brings up another point, can your Koha server send mail? Chris
