[Koha] Opac login information on Import Patrons

Plano ISD is doing just this our header for the file looks like this for the
data we are importing - 
"cardnumber","surname","firstname","B_address","B_city","B_zipcode","B_phone","userid","branchcode","categorycode","contactname","contactfirstname","sex","dateexpiry","patron_attributes","password"

The cardnumber is the student ID number and we are importing a password for
each student by default - last name or some other data.
