[Koha] Urget help!!

Greetings,

I'm from Hawassa University a place 271Km from capital city Addis Ababa, Ethiopia. Currently we are moving to use Koha for our Library. I had upgraded from 2.2 to 3.0 on Ubuntu 8.04 and I was successful using the manuals for installation and upgrading from Koha-duke site. But when the librarians are adding items in to the database, it will show us "production mode - trapped fatal error" . the error in koha log file states that
