[Koha] Holds

The system preference 'emailLibrarianWhenHoldIsPlaced' is not really functional,
as far as I can tell.   The cron job to process the message queue will send out
patron notification, etc when a hold is filled, but does not enqueue a message
when a hold is placed.
I've submitted a bug/enh report here:
