[Koha] Library card numbers, branch entry

The spreadsheet was one I made up quite a while back to help Rosalie when
she needed a few numbers because she was waiting for barcodes.  I didn't
have the formula in front of me, so I reverse-engineered it and it may have
some shortcomings.  But the numbers I tested it on worked.
