[Koha] disallow hold requests to be placed on items that are not checked out

Hi Liz and Justin,

the decribed behaviour is currently the expected one, since Koha appears
to always have worked like this.

As soon as one item is checked out, the record level hold is 'unlocked'.
It works ok for item level holds - items available can't be placed on hold.

There are several bug reports asking for a change of behaviour here -
one with others linked to it is:
