[Koha] Duplicate records showing up after z39.50 import

Hi everyone,
I'm using Koha Version 3.00.01.005 on Debian.
We've been trying to re-catalogue some of our books. To do this, the cataloguer brings up the current record in the catalogue, deletes all items, then deletes the record. He then searches for the book through z39.50 and imports a new record. After making whatever changes are necessary, he saves the record, then adds an item.
The problem we've had is that duplicate records with no attached items are sometimes (not always) showing up. These are not the old records stripped of items - they are duplicates of the new records from z39.50, always without items, and sometimes missing quite a few of the MARC fields (ex. in one case there were no fields above 3xx).
I turned on the cataloguing log, and it looks like a duplicate record is being added just when the cataloguer adds a single new record:
2010-03-10 20:28:04 31 CATALOGUING DELETE biblio 6158
2010-03-10 20:33:12 31 CATALOGUING ADD biblio 6160 biblio

Anyone have any ideas about what might be causing this?
