Without doing a step-by-step trace, I'm not sure why that is.

I don't know the vmware image. If it's a dev install, it might
be possible to upgrade it with git, but that would be very fiddly
without internet access (messing with pack and patch files).
Do any koha developers remember fixing a "No copies are available to
be placed on hold" bug which might cause the above problem?
