[Koha] Unable to add patrons

Kia ora,

I have just started using koha and am having trouble adding patrons.
Although I am am able to add biblio entries, when I try to add members, the
details are not recorded. Upon saving, the membership record is completely
blank.

I couldn't find an answer to this problem in the old posts or in forums
elsewhere. If anyone can help I would be most grateful...
