Check the permissions on /etc/koha.conf - the user running the CGI scripts (www-data) can't read it, so it doesn't know the koha username or password. IIRC, you probably want it to be -rw-r----- root www-data (chgrp www-data /etc/koha.conf ; chmod 640 /etc/koha.conf) unless you are running apache suExec or similar.
