I am having this issue also. I am running Koha 3.0 on Linux. It happens when I have a duplicate record and I delete one record. The catalog will still show both records and cause the patron to get the Error 404 page. Is there a way to prevent this.
Reindexing is not happening. Maybe: 1) You are searching for the record too soon (if you set the cronjob to reindex every minute) 2) You didn't set the cronjob properly. Tell us a bit more about your setup. To+
