We do the same, we have the ILL framework with the defaults already filled in, so we type the title in the bib record, scan a barcode in the item record and place the hold. (Make sure the item is marked as not being able to be seen in catalog in your default framework.) For returns, we just bulk delete the item records under tools and have a "sweep" deletion of all MARC records without items to clean out the catalog by our Vendor every month. (This works niftily for weeding deletions as well.) I had a Windows macro that autopopulated the fields (title, barcode, patron placed hold) from our Excel spreadsheet, but it is very unwieldy and not ready for distribution (I am not a programmer.) Please note that "Koha" is not an acronym, it's a regular (Maori) word.
Right now it looks as if the best method would be to create a fast add item for each title.
That's basically what we do, although we don't do it on the fly at the circ desk, since our workflow has been established since before the fast-add on circ functionality existed. We have a brief MARC framework set up and the ILL librarians add each title as they arrive. Our biggest problem is getting the ILL librarians to delete the records after the loan has been returned.
