Some things to check - were there any error messages in the Apache error log? Were all of the patron categories mentioned in the input file defined in administration before you did the import? If that doesn't help, perhaps you could create a sample file containing a couple patron records in CSV format -- anonymized, of course -- and attach it to a new bug at bugs.koha.org.
