Why are you getting the 'table does not exist' problems? Did you verify that the database was created properly and has all the tables? The koha installer does not do this, you'll need to do this yourself with a command like mysql -u <user> -D <databaseName> -p < /path/to/extracted/koha/installer/data/mysql/kohastructure.sql followed by logging into mysql and checking if around 116 tables were created in the database.
