We found the same problem, and follow the instructions in the CONSER Editing Guide, by using a 580 note with the combination of 785 16 fields, since the first indicator "1" in the 785 field suppresses the note from being generated from the 785 field in the presence of a 580 field. From the CONSER Editing Guide (785 section): 6 Split into … and … When this indicator value is used, input a 580 note. With second indicator 6, some systems do not generate notes from field 785 so the use of a 580 field is necessary. The first indicator value in field 785 is set to "1."
