I think I am making some progress. Now a message appears in the message_queue table but it is has a status of failed and not email is received. Any ideas what may cause this? I don't see any indication of a mail message being sent in mail.log or mail.err.
Have you had a look in the mail queue table? It should have all the generated emails, and have them flagged as to whether they've been sent or not. Also, if it's empty, that's interesting too.
