[Koha] permission problem upon upgrade from 2.2.9 to 3.04

Dear all we faced a new problem after upgrading from koha 2.2.9 to 3.0.4 and it is a permission break. whenever we create a new patron for one of our libraries in its default setting, this new account has a full privileges on all staff interfaces.
