[Koha] Mistake in the name of Live DVD manager name in 3.12.x release notes

Dear Friends, Name of Koha 3.14 Live DVD Manager name mentioned in all release notes of Koha 3.12.x. Another thing is, Live DVD Manager name has not mentioned in About Koha > Koha Team.
