[Koha] Renewal Problem

Hi, I find some problem with renewal. when I try to renew the books currently on issue, some of the books get renewed while others do not. This works fine some times, but not always. Can anyone please help me to solve this. If I have to check the code, which file do I look into. Please help me out of this. I am in real trouble.
