[Koha] OPAC User Can Not Login to Koha

      Hi all,

As $Subject said, OPAC user fails to login to Koha (v. 3.8.1) through
form login on Catalog main page or on OPAC login page[0]:

This is the koha-opac-error_log
(/home/subdomain_name/public_html/cgi-bin/var/log/koha-opac-error_log)[1],
and using MySQL Server version 5.1.63.

Any suggestion?
