It appears you need to compile PHP with MySQL support.
Are you referring to the documents that Joshua announced in this message: http://lists.katipo.co.nz/public/koha/2004/007207.html If so, those would be pretty old; more recent installation documentation can be found in the documentation section of koha.org and the Koha wiki <http://wiki.koha.org>.
