I've looked in Bugzilla and printing with RTL languages like Arabic should be possible: *Bug 12068* <https://bugs.koha-community.org/bugzilla3/show_bug.cgi?id=12068> - label-create-pdf.pl Add support for RTL languages (closed fixed) Maybe the discussion there could give you some hints.
