[Koha] Wiki account requests - now working

Account requests for the Koha Community Wiki are now working (
https://wiki.koha-community.org).

Following the Wiki upgrade last year, some work was required to get the
email notification working. This was resolved and working again from about
mid-December.

If you unsuccessfully tried to request an account after the Wiki upgrade
(and up until about mid-December), please try again.

Notes:
- New account requests are emailed a confirmation request - please check
your spam folder if it doesn't arrive within a reasonable timeframe.
- Please include some information in your profile: it doesn't have to be
much, just enough to help us separate you from spam requests (such as you
are interested in Koha, or your organisation uses Koha).
- Confirmed new account requests are moderated by selected community
members, so there may be a delay before your account is active: at most
this is normally done within a day or so (moderation is used to minimise
spam registrations).
