I forgot to mention that I also had to use chown to make that directory
owned by the user associated with the instance:

  mkdir -p /var/cache/koha/INSTANCE/templates
  chown INSTANCE-koha:INSTANCE-koha /var/cache/koha/INSTANCE/templates
