[Koha] Koha 16.05 Released

Version numbering changed. There is no more version 2 vs 3 vs 4 etc.
Instead, the version numbers now reflect the year (16) and month (5 or 11)
of the release.

So the next version after 3.x is 16.05, followed by 16.11. Pretty soon
there will be a 17.x, where the "x" will be the month when it is released.
