[Koha] Koha giving frequent software errors

Hello everyone,

We are using Koha for our resource centre and we are getting frequent software errors while working on it. Errors are like -

Error 1 :
Software error:

Wrong CSRF token at /usr/share/koha/intranet/cgi-bin/members/memberentry.pl line 291.

For help, please send mail to the webmaster ([no address given]<mailto:[no%20address%20given]>), giving this error message and the time and date of the error.
Error 2:

This site can't be reached

The connection was reset.

Try:

  *   Checking the connection
  *   Checking the proxy and the firewall<data:text/html,chromewebdata#buttons>
  *   Running Windows Network Diagnostics<javascript:diagnoseErrors()>
ERR_CONNECTION_RESET

Today, I got error when was working on Koha and to check something I clicked on 'Help'. I checked the Intranet error logs file and a snapshot of the same is below. The highlighted error is the error which was logged today when I clicked 'Help'.

Can anyone please suggest why these errors are occurring? Any help would be greatly appreciated because these errors are interrupting our work.
