[Koha] SQL for overdues / fines by borrower ?

Hi all,
One of my Liberians is asking for a fine report "by borrower" that can be
easily printed.

ie:

Borrower ID (xxxxx) Last Name, First Name:
              overdue_Item     ---- Fine
              overdue_Item2   ---- Fine

BorrowerID (xxxxx) Last Name, First Name:
              overdue_Item3   ---- Fine
              overdue_Item4   ---- Fine



this way we don't have to use 1 sheet of paper per fine, and sometimes wind
up giving the same patron 3 sheets of paper....

Wondering if someone already has this report made (and would be willing to
share)
