We are getting the following in our error logs when we are attempting to upload files into the record import management tools.  So, Koha looks like it is uploading our DAT file, but then we don't get the form to complete the import and the file isn't really uploaded to the server.

In addition, we seem to be missing a file... can we get a copy of just the blue.css?  Seems to have spontaneously combusted.
