[Koha] Koha live dvd 3.14.00

It worked for our small (5000 bib, 14,000 item) library, too. I changed
the default passwords, update as necessary, and everything's tickety boo.
Being behind a hospital firewall makes me worry a lot less, too.

(Which is all I'm going to say on this go-around.)
