[Koha] Koha Cataloguing Error

Will be grateful for help in resolving this error when adding a new item.
The following fatal error has occurred:
MARCXML document has no record element at /usr/share/koha/intranet/cgi-bin/cataloguing/additem.pl line 98.

Hello Members,
I keep getting error on koha whenever i add new record, 
Btw, i have checked my marc framework structure and it is all OK! Not sure what else could be the problem. Any help will be appreciated.
