[Koha] Koha Multiple Instances and Subdomain Site

Hi all,

How to make a subdomain site (ex: catalog.example.com) is able to take
advantage of Koha Multiple Instances[1] ?

What configurations must be modified/added (beside Apache virtualhost
configuration)?
