[Koha] How to enter the data

Hello, 

I am very new to koha, 
I can access both OPAC and Intranet in my browser. 
But when iam trying to add a member,it showing that the entered card number
is not correct. 

and 

Is their any date format specifications for it. 
if yes let me know which format should i need to follow. 

Please help on this.
