I have seen something similar when the index is 'off'.  Often a rebuild of the index clears this up.  

i.e. perl rebuild_zebra.pl -b -x -v -r

Try this

SELECT itemnumber,barcode FROM items WHERE items.biblionumber NOT IN
(SELECT biblionumber FROM biblio)
