[Koha] Koha Manual Updates

Hello all,

I have finally integrated the contents of the Wayne State U 3.0
manual, where relevant, into the 3.2 manual.

You can see the updates in Git:

soon on the web.

Next up I'm going to integrate some docs sent to me directly, if you
have comments or suggestions send them my - and as always feel free to
send me any patches you may want me to push to the manual.
