At the very least you will need to set an absolute default circulation rule.
This rule should be set for the default itemtype, default branchcode and
default patron category. That will catch anyone who doesn't match a specific
rule. Patrons who do not match a specific rule will get blocked from placing
holds, since there was no baseline number of holds to utilize.

Assuming one uses the documentation (impediment #1), maybe the confusion
arises with the use of the expression "absolute default circulation rule."
 That certainly confuses me.

Also, my understanding is that you have to have the so-called absolute
default circulation rule (aka default default) set or no one can place holds
even if the other rules are set.

Anyway, clear to me that this is a source of confusion that would benefit
from another round of wordsmithing or tightening up (on screen and in
documentation).  I'd be happy to help but I still don't understand really
how it works.
