[Koha] MySQL and utf8 in the installation instructions

There is a fairly comprehensive guide to setting the locale to utf8 at http://wiki.koha.org/doku.php?id=encodingscratchpad.  It is mentioned in Section 1.2 of the INSTALL.debian file, though curiously it is missing in the INSTALL.debian-lenny file...

Just a thought but something that has bothered me about the 
instructions for installing Koha.  In the instructions your pointed to 
warnings about installing with utf8 and no real approachable guidance 
on how to ensure you create your Koha database in MySQL as utf8.

Wouldn't this addition to all the instructions fix that problem and 
alleviate the need to do extensive reading on MySQL character sets?

create database koha character set utf8
