Per #1 -- do you mean this bug?

Per #2 -- this is an enhancement I have heard is in the works
(http://wiki.koha.org/doku.php?id=en:development:rfcs3.2:create-item-when-ordering).
 Right now, libraries using Acq are adding an item to their recieved
items and marking it as 'on order'
Per #3 -- I completely agree
