Yes, I want it in the debian distribution, so there's an installation
method (called standard, IIRC) which will follow the File Hierarchy
Standard when it works.  My koha.git tree has a branch called
"debianised" but I've not worked on it in a fairly long time due to
paid work taking priority.

Yes and they're almost there.  See recent posts on koha-devel.

This is always possible.  install-CPAN.pl tries to do it too.
