Manillah --

C4/Interface/CGI/Output.pm is an outmoded reference.  In 3.0, it is just
C4/Output.pm.  I would recommend using the 3.0beta version of Koha if you
can, or locate (and update) the references to Output.pm in whatever older
version it is you have installed.

Note: I'm unfamiliar with the configuration that has C4 installed under the
site_perl directory, and I suspect that might cause conflicts if you install
the newer version.  The current style is to use SET_ENV PERL5LIB in your
Apache configuration (or equivalent in shell), and leave the C4 directory as
part of the koha tree.
