[Koha] cannot add biblio item to Koha 2.2.9: error in Biblio.pm line 1440?

I am having a similar problem. I can do the z39.50 search but then get
similar errors when I try to add biblio and go to items.

I fixed this part of the the error: 

[Sat Jan 19 01:52:42 2008] [error] [client 206.248.178.11] File does not
exist:
/usr/local/koha/intranet/htdocs/intranet-tmpl/default/en/includes/intranet.css

by copying the intranet.css file from 

/usr/local/koha/intranet/htdocs/intranet-tmpl/npl/en/includes/

into the 

/usr/local/koha/intranet/htdocs/intranet-tmpl/default/en/includes/

directory

Now I can't get past the cryptic error:

The biblio fields get populated but when I try and add to items, I end up
with a server 500 error and the error at line 1440 in addbiblio.pl that so
many other people have posted about.

I have 2.2.9 installed on Ubuntu 6.06 LTS.
