I suspect your errors may be related to using MySQL 4.1. I know Koha is not compatible with the authentication in MySQL 5, but I wonder if they actually made the authentication change in 4.1.
If you want to try a 4.0.x version of MySQL, you can find it at http://downloads.mysql.com/archives.php?p=mysql-4.0 I would recommend the mysql-4.0.26-win32.zip download. That is the one I am using and it works. They have a 4.0.27 download, but it does not come with an installer.
If you can figure out what to do to get it working with MySQL 4.1, please let us know.
