Jenny,

It certainly looks like you are encountering the problem that exists 
when installing koha 2.2.5 and using MySQL 5.

Technically, this problem occurs because a particular table name used by 
Koha is a reserved word in MySQL 5 where it wasn't in MySQL 4.  The 
technical fix is relatively easy, the table name (returns? possibly) 
needs to be quoted in the db setup script.

Your options as I see them are three:

1. Install/use MySQL 4
2. Wait for koha 2.2.6 which has the fix and will work properly with 
MySQL 5 (most probably)
3. Search the list for more tech details and fix the db setup script 
yourself, or obtain the fixed db setup script from another source (I 
attach mine, but it may be stripped by the listserver.  Also mine comes 
with no guarantees, it worked for me but YMMV.  To use it, replace the 
koha.mysql in the koha 2.2.5 download with this one before you install)

As for using your old database, I have no idea.  I think the "add item 
type" problem is most likely a database problem though.
