[Koha] barcodes and importing marc

I'm having the time of my life trying to get koha to put items into the
catalog from an imported marc record. Below is the sample record with
which I am working. I tried doing it from Cataloging->Import Mark. When
I search from the cataloging module, I find the Marc record but there is
no entry in the catalog. 

Perhaps a separate issue: I also tried using the bulkmarcimport.pl
script, but it fails with 

DBD::mysql::db do failed: Access denied for user: 'kohaadmin at localhost'
to database 'Koha' at /usr/local/koha/intranet/modules/C4/Biblio.pm line
331.

koha.conf has correct settings, and everything else seems to work, so
I'm not sure where else I need to put my settings for db access.
