Right now this isn't possible, but there is a development to make all slips customizable - I'm not sure it will include the Routing slips, but it's a step in the direction you're looking for.

Hello!

 

  I am using the Koha serials module and was wondering if there was any way to add a location or address next to the patron name?  I’d love to be able to put a floor number/location next to it, if possible.
