[Koha] Patron Creation failure

Dear community

I have been trying to create a new patron under the category Juvenile with
no success.

This is the message i get:

"Something went wrong when creating the patron. Check the logs."

And when i checked the logs files at
/var/log/koha/library/intranet-error.log this is what i get:

I'm using the current stable release 21.05.
Please give me some advice.
