Hi David, 

I would like to help you, but please can you send some of your records / rows from the table where you find the duplicates ? 
Also after the erases from the problematic records etc. you said that do you try to continue the upgrade but with the same error ? if yes then we should check also for other tables in my opinion, if you can provide an sql to reproduce it somehow ( without records, authorities or members )  would be useful to find out what happened.

After you empty or delete the duplicate records , do you try to continue the upgrade ?
Either from web installer or cli ?
