Hi Dave, 

I had similar problem and I found that on the table  "language_descriptions" was the problem , because on bug 15607 is trying to generate unique indexes and if you have a double value there you have a problem like that you described. You should remove the duplicate value and re-run your upgrade though web installer. 
Also I suggest you to check for double values other language_* tables.

I have received this error when upgrading from 21.06.00.012  to 21.11. I do not see any information in the bug list or lists.
